Are you using not Redmine 4.x but Redmine 3.4.6?

I could not reproduce this situation with Redmine4.x, theme Circle and redmine_issue_templates.

And the latest version of issue templates which supports Redmine 3.x is 0.2.1.
Master branch does not support Redmine3.x anymore. (See: README)

I need two more information.

• Does this problem also happen under another theme such as "Redmine classic" or default?
• Could you give me the Redmine's information such as Redmine version, ruby version, database engine, web server and plugins including their versions you use.
  • You can use Redmine's information menu.
